The PHIP Annual Plan Change period is from October to November 15. We recommend you review your benefits annually to make sure you have the coverage needed for you and your family. Before October 1, PHIP will mail you a PERS Health Insurance Program Benefit Guide to review the upcoming plan year changes. Additionally, PHIP will hold Plan Change presentations in September and October to provide an opportunity for you to talk to PHIP staff and the Contracted Health Plans in your area.

If you are interested in changing your health or dental insurance plan for the 2024 plan year, you may submit a plan change request October 1 through November 15. You will need to complete both a PHIP Disenrollment Form and a 2025 PHIP Enrollment Request Form and submit to PHIP no later than November 15, 2024. Changes will take effect January 1, 2025. 2025 Enrollment Request Forms will be available no later than October 1, 2024.
If you do not want to make any changes to your health insurance plan for 2025, no action is required. Your current coverage will continue, and the 2025 premium rate and benefit changes will take effect January 1, 2025.
